The OHSA imposes duties on a supervisor and workers to report hazards and contraventions of the act and regulations. The Workplace inspection Form is used to record supervisor audits of workplace conditions and the identification of hazards or contraventions to the Act and prescribed regulations. Actions required or completed to address deficiencies and reach compliance must be recorded. Workplace inspections must be specific to a localized area or a specific workplace such as a heading or a building. They are to record all positive recognition opportunities and deficiencies. Hazards should have a good thorough description and attached picture. Immediate actions are to be recorded and any hazards that require follow-up action be assigned to the person responsible for the area with a proposed solution and reasonable timeline for completion.
