The Employee Discipline Form is used to record disciplinary actions address and document noncompliance issues relating to the Health and Safety Management System, Technica Mining’s policies and programs, company and site-specific standards and procedures, legislation and behavioural/conduct issues. The form is completed by the employee’s Supervisor and reviewed with the employee communicate the discipline, any penalties associated with the infraction and identify future expectations. Following the discipline meeting, the employee and supervisor acknowledge the review of the document by signing the form and the form is then sent to the Manager Once Removed for review and filing with Human Resources. The discipline guidelines set out in the company discipline policy are to be used as a reference when determining the level of discipline.
